feat: enforce rails in the environment, not the runtime 038f17a3af

Bitter Lesson diet, phases 1-3:

- Phase 1: push protection moves from per-adapter command parsing to a git
  pre-push hook (.scaffold/hooks/pre-push), installed by scaffold init/update.
  Same .scaffold/-only exemption and SCAFFOLD_OFF escape hatch; works for any
  runtime and any phrasing.
- Phase 2: closeout enforcement becomes a pure outcome gate. closeout-check.sh
  now passes when nothing shipped and checks log freshness against the last
  code change; both adapters drop their session-baseline machinery and just
  relay the gate's verdict. Claude Code loses the PreToolUse parser (setup
  retires stale entries); adapters are now injection-only plus gate relay.
- Phase 3: BOOT diet — prescription becomes information. BOOT.md drops to
  ~50 lines (hard rails, two imperatives, map); the FRAME→WORK→REVIEW detail
  moves to cookbook/loop.md, pulled on demand; ORIENT is rewritten as
  what-exists-and-why rather than a numbered ritual.
